Acupressure for Migraine and Tension Headaches: Therapeutic Potential and Evidence-Based Perspectives

Abstract

Migraine and tension-type headache are among the most common neurological disorders and represent important causes of pain, disability, reduced productivity, and impaired quality of life. Although pharmacological therapies remain central to headache management, concerns regarding adverse effects, contraindications, medication overuse, treatment costs, and incomplete symptom control have increased interest in complementary and non-pharmacological interventions. Acupressure is a non-invasive technique involving the application of manual pressure to selected points of the body and has been investigated as a potential approach for headache prevention and symptom management. This review examines the therapeutic potential of acupressure for migraine and tension-type headache, with particular attention to proposed mechanisms, clinical applications, evidence of effectiveness, safety, limitations, and future research priorities. Possible mechanisms include stimulation of peripheral sensory receptors, modulation of nociceptive processing, activation of endogenous pain-inhibitory pathways, autonomic regulation, reduction of muscular tension, and psychological relaxation. Clinical studies have reported potential reductions in headache frequency, intensity, duration, and associated symptoms in some populations, while evidence syntheses suggest that acupressure may provide benefits for selected patients. However, interpretation remains complicated by heterogeneity in pressure points, treatment protocols, session duration, treatment frequency, control conditions, sample sizes, and outcome measures. Distinguishing specific acupressure effects from nonspecific effects of touch, attention, expectation, and relaxation is also methodologically challenging. Acupressure appears to have a generally favorable safety profile when appropriately administered and may be considered as an adjunct to established headache-management strategies. It should not, however, replace appropriate medical assessment or proven preventive and acute treatments. Future studies should employ adequately powered randomized controlled designs, standardized intervention protocols, credible control conditions, objective outcome measures, longer follow-up, and cost-effectiveness analyses. Further research may establish which headache phenotypes and patient populations are most likely to benefit from acupressure and how it can be integrated into personalized, multimodal headache care.
